Kouper, a health tech company focused on improving transitions of care, has officially launched out of stealth with $10 million in funding. The round was led by General Catalyst, with participation from 25Madison, CVS Health Ventures, and several health care angel investors.
The company partners directly with health systems to improve patient transitions after emergency room visits or hospital stays, aiming to reduce the $53 billion annual cost of avoidable readmissions in the U.S.
The time immediately following hospital discharge is often the most vulnerable period in a patient’s care journey. Poor coordination during this stage leads to missed follow-ups and deteriorating outcomes. Kouper addresses this gap by offering a platform that uses generative AI to automate patient outreach, scheduling, and data capture throughout various care settings.
By consolidating fragmented information into a unified view of each patient’s journey, Kouper helps health systems make better-informed decisions in real time. The platform gives providers better visibility and ensures that care teams can act quickly and efficiently to prevent readmissions.
With this fresh capital, Kouper plans to expand its product capabilities and team, aiming to scale up its infrastructure to support more hospitals nationwide. The platform is already seeing strong performance—reportedly driving a 50% increase in patient engagement and over 60% more follow-up appointments post-discharge or ER visits.
The company will also invest in growing partnerships with health systems and risk-bearing entities. Its four-week implementation timeline allows partners to quickly realize value without major operational changes or hiring requirements.
Kouper was founded by Salman Ali, previously co-founder of GetSnooze, and Ablimit (PhD) Keskin, former engineer at Komodo Health. The company began as part of an incubation with 25m Health, combining startup agility with deep health system insights.
Ali, now Kouper’s CEO, says the company’s approach was shaped by lived experiences and direct feedback from early health system partners. Kouper’s model centers on creating personalized, concierge-like transitions that take into account each patient’s unique path across the care system.
General Catalyst, 25Madison, and CVS Health Ventures are joined by multiple health care angel investors in supporting Kouper’s mission. Each has expressed confidence in Kouper’s role in shaping a more coordinated care landscape.
"Kouper is solving one of the most critical challenges in health care..." – Chris Bischoff, General Catalyst. "It’s more important than ever for health systems to adopt Kouper’s comprehensive, AI agent-powered transitions of care platform..." – Merrill Anovick, 25Madison.
Early collaborations include partnerships with LifePoint Health, Henry Ford Health, and Ascension Saint Thomas, signaling growing institutional trust.
